We have all seen promising preclinical work with combinations which has resulted in a clamor and a scratching of heads as to why this apparently has not been pursued. There has been the Yale study, as well as combinations with Venotoclax for AML, and the Rux sequential study to name a few. There has been the ancient breast cancer combination therapy which unfortunately appeared to fail. When asked about combination therapy, Dr. Scarlett demurs and makes reference to the problem of toxicities, either additive or synergistic. The studies from Japan looking at esophageal cancer (and I believe there have been other studies also looking at Imetelstat/radiation in preclinical models), seem to be off the radar but unless we are talking about whole body radiation or large areas of radiation such as mantle radiation, the toxicity of radiation is local and for the most part intended. This is a combination therapy that might have minimal additive or synergistic side effects while improving efficacy and perhaps selectively target CSCs along with standard therapy. Just speculation at this point but someone appears to be looking into it. bp
