Re: For Cheng Ho

Hi Irish, thanks for your thoughts and your new article. I did review the Tefferi Italian talk. Cheng, I doubt the recent California presentation would have anything new since they were only a week or so apart. The article certainly confirms movement toward patient specific therapies directed toward...

Re: The MDS market

Sargasso, I believe your inquires require an Occam's razor approach:

1. You are wrong.

2. Janssen is wrong.

3. The market is wrong.

Pick one.

For Cheng Ho

Hello Cheng, How are things in sunny New Hampshire? I have a serious question for you. You have worked in the Shay lab and have a science background and have essentially forecast the demise of Geron. You have said it is an old drug still looking for a home. It appears that the holders of millions of...

The Math

I thinks its worthwhile to remember that only twenty patients were analyzed in the high does arm after three months of therapy--that is, they each at the time of analysis had received four doses of Imetelstat. 20 patients, four doses, the last of which might have only been in for a short week or so....

Re: Imetelstat Update: Some Thoughts

Sargasso, its not about PR or CRs which actually did occur in some Mayo patients within the first 12 weeks (see Geron investor presentation post ASH 2014 slide 19 I think), it was about some secret predetermined measurements that Geron and Janssen baked into the study to perhaps be a go/no go determ...

Re: Imetelstat Update: Some Thoughts

Irish, great collection of thoughts and observations. I agree with you on all counts. I thinks it important to review what we really do know. 1. The drug works for a subset of patients with MF and a subset of patients with MDS. Janssen/Geron knows that analysis of the 90 or so patients in the 9.4 do...

Re: Geron Provides Update on Imetelstat Trials Being Conducted by Janssen

Irish, can't tell you how glad I am to have you here to help sort through this stuff and to provide guidance. Two small issues though, one is that I think Dr. S comments re consistency with Mayo data only applied to MDS not MF. Also this failure to reach some prespecified 12 week goal was a little c...

Re: Geron Provides Update on Imetelstat Trials Being Conducted by Janssen

To keep things in perspective: Anna has been kind enough to confirm that the twelve week mark is for each patient. That means that no patient was analyzed in a way that reflected more than 12 weeks of therapy each. In other words its not an analysis that included every patient that was initiated twe...

Re: Geron Provides Update on Imetelstat Trials Being Conducted by Janssen

What Scarlett said, or didn't: 1. Enrolled folks have about six months to live on average. We'll take another look in about six more months. Shouldn't most of them be dead? We only looked at twenty patients in each arm anyway. 2. Nope, not enough spleen effect or symptom control to call it (but we t...

Re: Link to EU ODD mtg kindly provided by Anne YMB

Now Cheng, I believe you are confused. Try to take a deep breath and do serial sevens to be sure everything is working upstairs. The EMA documents of Sept 6th confirm a discussion of imetelstat. Taz simply made a mistake (he usually doesn't) and thought that an entry on a Facebook site was related t...

Re: Is Taz on here?

Irish, There really were some hurtful and insensitive folks on YMB and having a moderated board (thank you Fish!!) is a blessing since it was really hard without a library of information and an informed chief librarian (did I mention--thank you Fish!!). Irish, I don't know if you have been following...

Re: Was wondering what the various reasons are to continue the expansion of the CTs over the next year or more?

Hi Sargasso, sure. Let me explain. We have all seen studies that never panned out and were consigned to the "Journal of Irreproducible Results." Maybe the placebo group did better than expected or there was some fluke in the study design. Contrast this with total reversal or significant improvement ...

Irish, I am so pleased that you are back and John is doing well. I thought at least one Florida site was on line so you wouldn't have to keep traveling. I thought we didn't hear from you because of some "gag order." Best wishes for continued health for you both. bp

Re: Was wondering what the various reasons are to continue the expansion of the CTs over the next year or more?

Hello Hunt, I will try to give you my perspective on your questions. These are my opinions only. 1. Looking at failed Jakafi patients is a short cut to a niche and so enhances the road to approval even if in a small group of patients. 2. Once on the market however because of superior efficacy, the d...

Re: Sept 16 Tefferi talk

Hello Hunt, I believe you are correct. From everything I have learned, Dr. T is untainted and does not have a conflict of interest here. Look back at his presentation at ASH years ago. He was NOT present at the "independent" Geron review of his data the evening following his presentation. He has nev...

Re: Sept 16 Tefferi talk

Hello Cheng, I think its important to not be so mercenary about this. Conferences like this are for patients and families. It should be limited. If only 100 can attend and one seat goes to a financial type, that in itself is a tragedy. We should not lose sight of the importance of this information b...
